Walk-in/ Urgent Care Clinic:
This Clinic provides services for health problems that need urgent attention, but are not life threatening. To be seen in this clinic you do not need an appointment.
We treat illnesses like cough, congestion, allergy symptoms, bronchitis, ear pain, flu-like symptoms, sinus infections, pink eye, urinary tract infection, skin rash and STD. As well as, minor injuries like superficial cuts and lacerations, suturing of simple wounds, sprains and minor fractures. We perform examinations and treat minor work-related First Report of Injury for BWC, School Physicals, Work Physicals, and DOT Physicals.
For your convenience, we have onsite digital X-ray services and point of care testing, including, strep test, flu test, urine test, and pregnancy test.
Chronic Care Clinic:
This Clinic provides services for ongoing chronic medical problems. Patients are seen with appointment only in this clinic.This clinic is run by an experienced Board Certified Internist who has several years of experience in both in-patient and out-patient management of patients with complex and multiple medical problems.
In this clinic, we manage chronic health problems including, diabetes mellitus, high blood pressure, arthritis, COPD, asthma, congestive heart failure, high cholesterol and chronic kidney disease.
Experienced, trained, dedicated and caring support staff members are ready to help our patient in best possible way.
Transition Care: The goal of transitional care is to make post hospital discharge and follow-up easy and convenient for the patient. After being discharged from the hospital, patients often have questions about their illness and concerns about their new medications. Our dedicated and trained staff are available to make early follow-up appointments and answer any questions you may have.
Preventive Care: At CMC, we provide appropriate screening,counseling, and preventive care services for our patients. Many diseases can be prevented through close partnership with your healthcare team, or can be detected through appropriate screenings. We offer preventive screenings and education in a convenient and efficient manner. Our recommendations are based on the current, evidence-based recommendations of the U.S. Preventive Services Task Force (USPSTF). To get customized recommendations from the

Coumadin Clinic: Patients who are taking coumadin (Warfarin) need close follow-up of blood test. In this clinic we educate patient and manage anticoagulation therapy. PT and INR Test is done at point of care by collecting blood sample with a finger prick and test results are available immediately. Advice on dose adjustment if needed is given at the time of visit before patient leaves the office.
Outpatient testing: Screening for Obstructive Sleep Apnea, Peripheral Vascular Disease, Holter monitoring, EKG and Spirometry.
Phlebotomy Service: Lab draw services for blood tests is available for your convenience.
