Immigration Physicals
Community Med Center also provides Immigration Physicals for USCIS adjustment of status applications. All such physicals are by appointment only.

Documents Required:
All applicants are advised to bring a document showing their Alien number (A#) . This could be a notice of receipt from USCIS (commonly known as I-797) for their I-130, I-140 or I-485. It may also be an Employment Authorization document, Travel document or any other document with an Alien number.

A valid picture ID. This may be a valid driver's license, government or military issued ID, Employment Authorization Document, or a passport. Occasionally an applicant may not have a passport. This may happen in case of Refugee or Asylum applications. If this is the case, a passport is not required.

Any written vaccination (immunization) records.

Fees:
Immigration Physical Package $240.00: includes any/all of the following:

Immigration Physical Examination.
Syphilis test (RPR test)
T.B. skin test (PPD test)
Chest X-ray (If TB test is positive)
Tetanus Vaccine.This vaccine is needed for individuals who do not have a vaccination record.
Test for Measles, Mumps, Rubella immunity.
Test for Varicella (depending on history of previous vaccination or history of chickenpox)
For most applicants the above is usually sufficient to complete the exam. In certain cases, additional adult vaccinations might be needed (see below). Depending on other health conditions, treatment or referral for further testing might be needed. This is determined by the doctor at the time of the visit and after test results are back.

Additional Vaccination (additional fees apply, if these vaccinations are needed):
In accordance with immigration guidelines and based on your test results, the following vaccinations if indicated can be provided for an additional fee or you will be referred to a clinic of your choice.
M.M.R. vaccine:T
he MMR vaccine is given if indicated.For individuals born after 1956 and who do not have a vaccination record , and your test results show that you have no immunity to MMR.
Pneumococcal vaccine: This vaccine is needed if you are above the age of 65 and you do not have a vaccination record. 
Influenza vaccine: This vaccine is needed for individuals over 6 months of age who do not have a vaccination record for the current year. This requirement is applicable between the months of September through March.
Hepatitis B Vaccine: This vaccine is needed if you are less than 19 years and do not have a vaccination record.
Varicella Vaccine: The Varicella (chickenpox) vaccine is given if indicated. For individuals who do not have a history of chickenpox and blood test results show that you do not have immunity.
